Ellis Rogers' Book on Quadrilles

Jimmie Rogers' article on The Lancers in the RSCDS Bulletin for 2003
mentioned the upcoming publication of a book by Ellis Rogers called The
Quadrille: A Practical Guide to Its Origin, Development, and Performance." I
would think the book would be out by now, but I haven't found any mention of
it through a web search or the British Library catalogue. Does anyone happen
to know anything about it?
